You may not have received the case numbers or the date from the credit reference agency but they may have given you details of who the creditor was and the amount and the date of the judgement.
If this is the only information that you have then you should send this on to the county court instead of giving the case numbers.
Also note that if these judgements were made against you at a different address you will need to inform the county court that the judgement was made at another address and give them that address.
